Simply add "--ar W:H" at the end of your prompt, replacing "W" with the width and the "H" with the height.

When planning this article, the main objective was to test, from a presenter’s perspective, an introduction to ChatGPT for PPT presentations. Therefore, we devised a hypothetical case: A brand wishes to present its new smartwatch at a live event. The presentation has to be streamed, and there is a select live audience attending the event. This presentation follows the format of other tech presentations like new releases.

What are our main objectives to achieve in this case study?

  • Obtain an outline of what the presentation should be about.
  • According to the “sales presentation for a smartwatch” requirements, obtain a list of elements to include per slide.
  • Generate suggestions on how to engage the audience to buy the product.
  • Get insights on which elements to avoid while performing the presentation.
  • Repurpose the presentation’s content to another format (article, social media, infographic, etc.)

So, our case study starts after logging into ChatGPT and giving detailed instructions: “Create an outline for a sales presentation about smartwatches.”

Interestingly enough, it’s time to cross-reference with what we, as presenters, intend to create out of the presentation.

Slide #1 – Introduction

As suggested, the ChatGPT introduction PPT will be handled through the first slide. Since the brand presenting this smartwatch is already established and has brand recognition, the purpose of the presentation shall be to track back to some key features of prior presentations. This implies if the brand has an iconic aesthetic used to start the presentation , that format should be preserved.

The point about the importance of smartwatches in today’s world is rather obvious given the kind of event; therefore, it will be replaced with some stats on how the brand has helped customers through the years to improve their lives thanks to these devices.

After the introduction is made, it should be immediately followed by an agenda slide .

Slide #2 – Benefits of smartwatches

Instead of generalizing the benefits of smartwatches, this second slide will directly speak about the features to be introduced in the to-be-released unit. It has to mention updates on health and fitness tracking functions (i.e., sleep pattern recognition, medication reminders, blood oxygen, wrist temperature, etc.), which new integrations it offers for mobile payments, productivity tracking apps, and how the notification system is updated. For convenience, we can directly mention a lighter watch specification (in terms of size and weight) or include those elements in the third slide.

Now is the time to display your product in front of an audience. Some high-key photos taken by professionals should appear, as well as animations with a 360-degree view of the model.

Call-to-actions are to be introduced here, using the element of excitement to create interest in buying the product.

As we can see, ChatGPT’s approach isn’t far from what we accommodate the presentation to be. It is pretty generalist despite mentioning the case in first person during the user input, but that can be expected from AI deep learning models. Reiterative answers in some sections can also happen, although not as noticeable as other alternatives to ChatGPT, which we shall mention later.

Engaging the audience with ChatGPT

Points #1 and #2 of our case study are done, so we move to get suggestions from ChatGPT to engage the audience about buying the brand’s smartwatches.

This AI tool presents us with six different hooks to engage our audience .

  • Pain point focus : We have to identify a common pain or problem many people face, then show how our smartwatch solves that problem. For example, it uses the case of difficulties experienced in time management and how a smartwatch can solve that problem.
  • Personalization : This hook focuses on how the smartwatch can be customized to fit each individual’s needs and preferences. We can mention changing the watch face, the app market, or using custom bands. 
  • Health and wellness : By highlighting the health and fitness tracking capabilities, you can show customers how to lead a healthier and more active lifestyle.
  • Convenience : This hook points to using the smartwatch in daily activities such as making phone calls on the go, enabling mobile payments, or browsing notifications without needing to check your phone.
  • Emotional connection : Using customer testimonials, you can create personal stories demonstrating a smartwatch’s positive impact on someone’s life.
  • Highlighting unique features : This would be your competitive edge. Explain which features make your smartwatch stand out in the market.

Recommendations on presentation mistakes

An advantage of GPT language is the ability to develop accurate recommendations based on the context showcased. For our case study, we asked ChatGPT if there were any mistakes to avoid when giving this presentation about the release of a new smartwatch.

In this case, ChatGPT presents us with seven mistakes that presenters commonly make on topics like this. We can put aside the lack of preparation, as no professional presenter would arrive at a product release presentation without proper practice and assets. 

The second mistake is related to bombarding your audience with technical details, which can end up boring the spectators. Instead, it offers the alternative of shifting focus to the product’s benefits and how it can help solve real-life problems.

Ignoring the competition as a mistake is questionable. You don’t see Apple speaking about Samsung smartwatches in their product releases, nor Sony talking about Nintendo or Xbox when presenting their newest PlayStation unit. That should go more from a preparation side when researching what the competition did to catch the audience’s attention.

Not showing the product! This suggestion surprised us. Honestly, how can you come up with a presentation about a product release and not even show a photo of the product itself?

Failing to answer questions can happen if the presentation stretches long enough to conclude the event, but it is not a common mistake in professional technology presentation events.

Poor visuals and slides, yes, we concede it is a common issue experienced by presenters, especially if they are overly populated with text.

Finally, it lists not practicing the presentation, which we also put aside as we did with the first mistake.

Overall, we ended up with 2 valid suggestions to look for out of the seven suggested. We can always retry the output, but when we tested that, the suggestions were pretty much similar.

Repurposing content with ChatGPT

So what if we intend to use the information in this presentation for another medium, for example, a blog post on the brand’s website ? We asked ChatGPT to give 10 title ideas for blog posts on smartwatches for a tech website.

  • “The Top 10 Features of the Latest Smartwatches”
  • “Why Smartwatches Are the Future of Personal Technology”
  • “How to Choose the Best Smartwatch for Your Lifestyle” – OK
  • “The Health Benefits of Owning a Smartwatch” – OK
  • “The Most Innovative Smartwatches of the Year”
  • “Smartwatches vs. Traditional Watches: Which is Right for You?”
  • “Maximizing Your Productivity with a Smartwatch” – OK
  • “How Smartwatches are Changing the Game for Fitness Tracking” – OK
  • “The Convenience of Mobile Payments with a Smartwatch” – OK
  • “The Impact of Smartwatches on the Wearable Technology Market”

Out of the ten titles suggested, half are a good fit for the content of our presentation, which, of course, requires additional research. 

Asking ChatGPT for extra recommendations

So far, the outputs generated by ChatGPT have been interesting from a content perspective. But let’s ask some more questions to test its true capabilities as AI content.

We start by asking, on that same thread, how long the presentation should be.

It gives a vague answer about sales presentations, recommending sticking it below the 30-minute mark. The section about “clear, concise, and engaging manner” is presented as a vice through this thread – we believe we counted about 7-8 times that same sentence repeated in the entire chat.

We move on to another question, this time asking ChatGPT which would be the best visual medium to present a smartwatch: a detailed infographic or a video.

Again, we see the “clear, concise, and engaging manner” phrase in the second paragraph, which is somewhat rephrased in the final paragraph of this output. The suggestion retrieved is that infographics can help sales presentations highlight key features and benefits while also clarifying to the audience what your competitive advantage is. It recommends using videos as a bridge for emotional connection with the audience since they can demonstrate the product in action.

Finally, we wanted suggestions about the color scheme in the presentation slides to encourage customers to buy the product. Below is an image of the output generated.

If you check our article about color theory , especially in the section about color psychology, the suggestions are okay. The question lies in a generalist output rather than explicitly suggesting a color palette for this.

Now it’s time to brief some tips to help presenters extract the potential of ChatGPT for presentations at its current state-of-the-art.

Avoid complex inputs

Asking a detailed question can make ChatGPT crash. You can question whether it is your mistake or just the server overloading. A good practice for this is checking ChatGPT’s homepage, and if the message shown below is displayed, there’s a server outage.

This can be flagged as a partial outage triggered by requests going way higher than the system can handle. Those peaks in navigation are seen at certain hours, especially during the Pacific Time afternoon. A complete server outage can happen, and that’s notified through a different warning, saying the service will be up soon.

Don’t rely on the text generated by ChatGPT

Especially if your intent is to repurpose the presentation’s content for a blog post, be EXTRA cautious when using AI tools like ChatGPT. Google is flagging and penalizing websites that retort to AI content generation . This is explained by the necessity of providing users with the best quality content available for their search queries. Your website can endure a severe demotion in rankings triggered by ChatGPT content.

A good practice is to rephrase the content manually, meaning to actually read what ChatGPT generated and explain it in your own words. That way, ChatGPT becomes an idea generator rather than a content generator.

Take a look at repeated outputs

You can always regenerate the response if the output doesn’t satisfy your needs or rephrase the question to make it more specific. We tested the option of saying “do not include…” but it works randomly. 

Any AI tool for presentation is not intended to become an instant solution, and we cannot assume that we can entirely create a presentation with ChatGPT as the only resource. This process requires patience, skills to give the proper instruction (to avoid those repeated outputs), and a good deal of copywriting skills.

Outlines and title generation

ChatGPT is an amazingly effective tool for creating presentation outlines, helping a common writer’s block. Title generation is accurate, although it falls into the generalist category and can be easily flagged as AI content. We advise you to double-check titles, rephrase, or simply use the output for brainstorming.

Learning a new skill/definition

Say you have to deliver a presentation about a concept you don’t clearly understand – which, under some situations, can perfectly happen. ChatGPT can help you learn that concept to basic professional competence, and you can ask as many questions as desired to get a broader picture of the topic. Think about it as a personal, free coach. 

Of course, do not take the outputs as absolutely true, as some responses can be biased through the machine learning procedures applied. With dedication, you can train ChatGPT yourself and help the community get better responses.

Evaluate emotions

Yes, as cutting-edge as it sounds, the natural language processing features of ChatGPT and its machine learning algorithms help users evaluate their emotions in different datasets. How can this process be accomplished? Let’s go through a basic overview.

  • Data Collection : Feed ChatGPT with transcripts from speeches or customer reviews.
  • Model Review : ChatGPT uses machine learning algorithms such as Support Vector Machines (SVM) or Recurrent Neural Networks (RNN) to develop the emotion recognition model.
  • Prediction : After the system processes the information, the output is sent using predictions that can be repurposed into new information.
  • Integration : With the new information generated, we can create graphs and charts to turn into visually-understandable data the emotion analysis generated.

This kind of feature is ideal for processing large chunks of data regarding customer reviews and finding common patterns of conflict (i.e., areas in which customer service can be improved).

Despite being the best-advertised option for GPT language tools, presenters should check other AI alternatives for content generation.

In our experience, Jasper one of the best content generator based on AI available in the market. We tested it in multiple scenarios, like generating metas, creating a presentation structure, generating step-by-step solutions, title ideas, and more.

Our objections? Jasper can be reiterative with its answers, rephrasing or reusing the same sentences in the L length (which is the maximum) of text generation. Much like what happens with ChatGPT, some answers fall into the generalist side, meaning there’s room for improvement.

We can appreciate considerable differences in keyword management for content generation (Jasper only allows 3 keywords), maybe due to this tool’s primary intent, which is web content.

Users can generate presentation structure, content briefs, large pieces of content, detailed analysis, and more, and customize where the AI engine is extracting the information from (if SERP, People Also Ask, Questions, Headers, Topics, etc.)

For presenters, Frase can be used to generate speeches or repurpose presentations into quality blog articles. 

Tome.app ppt is a new multimedia tool to generate AI-powered presentations by integrating ChatGPT, Dall-E, and other interesting features.

The free mode offered by Tome is pretty simple. You sign in and then are offered a set of pre-made templates to test. Or, you can hit the create button and create a presentation from scratch. 

You then write what the Tome presentation is about and click on create. Options for selecting which type of illustrations you want to add are listed, and if you intend to create a presentation, an outline, or a story.

It is an interesting take on presentation crafting. However, some users can feel it is restrictive due to using only Dall-E-generated images instead of uploading their photos.

Finally, we have iA Presenter, a tool still in the beta stage with a waitlist to access its features.

The proposal is truly interesting: you introduce text that can be extracted from multiple sources, and then you designate which are the headlines (using the # in front of the text). Slides are separate from each other by adding a – – – (triple dash) on the text. 

Images can be easily incorporated, and there are plenty of options to create visual cues, such as gradients, which are better audience magnets than common bullet points.

In this section, we wish to list some of the tools we have tested to speed up the content creation process from scratch. Yet when we consider the future of AI presentation as something vague, these tools have caused an uproar in terms of content development.

As the final AI image generation tool on this list, we should mention Midjourney , an AI tool that has raised controversies when winning an artist competition against real-life artists .

To access Midjourney, you have to apply to the official waiting list. After you are accepted, you’ll receive an invitation to their Discord server – a requirement to interact with the tool.

Like any Discord server, your interactions with the tool are public unless you create a private channel and invite the Midjourney bot to it. This would be our personal suggestion so you preserve the privacy of your projects. Midjourney works with credits in a subscription-based plan (basic tiers allow 200 images per month). The time to process each image depends on demand.

As we have seen with Jasper, DeepAI is a text-generator AI tool based on NLP (natural language processing). Its application is to create creative content as if written by humans, but we found its most powerful application in text translation.

We can all agree that video content production is expensive. Especially if we have to hire actors to do voiceovers or have them appear in video presentations. The current state-of-the-art for AI presentations is rather incipient. 2022 can be taken as the first year in which AI started to fill the conversation topics and proved itself to be a powerful variant of human-created content, although not a replacement.

We consider there’s room for improvement in ChatGPT for PowerPoint presentations and in all tools presented here, which naturally shall follow through usage. Regarding ChatGPT, the potential lies in idea generation, not copying/pasting extracted content. Think of ChatGPT as an extra tool in your arsenal to become a better presenter, not the panacea for presentation design.

Getting the most out of this impressive tool is not that different from any other powerful system, it's about playing around and seeing what works. Unlike learning Word or even a new game, learning ChatGPT is just a case of talking to it — it can even tell you what it can do.

Each new message with ChatGPT isn’t like a new search in that it's one hit, get your answer and move on  — each chat is the start of a conversation and this feature can be used to create interactive stories, and games or even work through complex problems step-by-step .

Vision and code analysis features in ChatGPT can also be used both on their own or as part of a bigger project. For example, you can share photos of furniture you want to buy and have ChatGPT create a color scheme for your walls that matches the aesthetic.

Tips for getting the most out of ChatGPT

The best piece of advice I can give for using ChatGPT is to pretend its a human. Speak to it like you would an assistant or tutor. If it helps you can even open a conversation by telling it to take on the role you need. For example, “You are a French tutor helping me learn the language.”

Another tip is to know the tools within the platform. For example, if you want to debate an idea then you’d be best switching to voice mode (available in the mobile or macOS apps) but if you need a specific type of output such as a graph or table use the text chat.

If you’ve got ChatGPT premium you can use DALL-E to generate images and in both the free and paid versions you can upload pictures or documents for deeper analysis. Finally, if you find yourself hitting the limit of messages with GPT-4o, switch to mini — it’s still a powerful model.

If you’ve got a large block of data you can have ChatGPT generate a graph or chart using built-in data analysis features It can create bar charts, line graphs, pie charts and more and can add extra style if you ask it during a follow-up message.

Even if your data isn’t particularly well organized you can still use ChatGPT to visualize it. If you’ve got a PDF of a report you can load it into ChatGPT and ask it to pull out key data points and turn it into an interesting visual. 

You could even feed this into a DALL-E image if you have ChatGPT Plus although this won’t be as accurate as just using data analysis to make a graph.

Example prompt: "Go online and find a report on cat ownership in the US and visualize it with a graph." The result will be a breakdown of data and a basic graph. You can ask it to "make the graph more engaging" for a better-looking image.

OpenAI is building a new tool called SearchGPT aimed at improving the way search works inside ChatGPT but while you’re waiting the current version can search the web.

It is fairly limited but can be good for finding specific data points — just double-check everything as it still hallucinates and will sometimes just take data from its training.

To know whether ChatGPT searched the web look for the word ‘searching’ at the top of its response. Also, use words like “search the web” “go online” or “get up-to-date information” followed by what you want it to look for.

ChatGPT is a brilliant editor. It's a better editor than it is a writer as often it will lean towards being overly formal in its writing style, but it can turn even the clumsiest of copy around.

While all of these features are available in most modern text editors, or at least will be soon, it can refine your writing, correct your grammar, suggest style improvements and even enhance the tone to suit a different audience — just be very clear in your instructions.

Many of these features will soon be integrated into a device, whether through Copilot in Windows 11 or Apple Intelligence on a Mac but for now copy-pasting it to ChatGPT is a good solution, and an alternative to paying for Grammarly Premium — although that is also a good option if you want to improve your writing more naturally.

When I was in Paris recently I was able to give ChatGPT a photo of a food label written entirely in French and not only have it translate the contents into English, but also highlight items that could trigger an allergic reaction or don’t match my own food tastes.

You can also turn on transcribe and have it listen to someone speaking another language, have it translate it to English and do the same in reverse — acting as a conversational aid across linguistic barriers. Or it could just help improve your own French, Spanish or Klingon.

In school, math problems often seemed to involve working out what was going on with a train traveling between two cities or how a group of people could share a bowl of fruit. This was just a way to demonstrate a real-world version of an equation and ChatGPT can do something similar.

Let’s say you need to know how tall a tree is for a planning application. We’d turn to basic trigonometry and get the angle of the tree, our eye height and our distance from the tree. With ChatGPT, you could just give it the numbers and have it crunch it all for you.

You could even ask it how to calculate the angle to the top of the tree and have it give you dummy numbers so you have an idea of what your measurements should be like. At every stage, it can offer up step-by-step directions.

ChatGPT

ChatGPT isn’t as good at video analysis as Google Gemini Pro 1.5 but it can do it and does so by breaking it down frame-by-frame and pattern matching. GPT-4o can, in theory, analyze an entire video natively, but that feature hasn’t been enabled by OpenAI yet.

After it goes through the video ChatGPT can provide a summary, extract key points and generate transcripts from the audio element of the video file. You can also have it identify important moments within the clip through its frame analysis.

ChatGPT’s image analysis features, especially when using GPT-4o are incredible. It can pick out even the smallest detail of an image and for this reason, it is a great way to create a recipe.

Upload a photo of your fridge or a pile of ingredients on a counter and ask ChatGPT to make you a recipe that uses all or some of those items. It will work out the quantity you have available, determine the best flavor match and offer a meal — in theory.

The better the ingredients the better the recipe. I once gave it a can of peaches, pizza dough, and mozzarella cheese and it suggested a peach pizza.

ChatGPT is a large language model (LLM) trained on over 400 billion words from books, articles, and websites. Its extensive training draws from a large database of information, making it valuable as a diagnostic aid. Moreover, its capacity to comprehend and generate human language allows medical trainees to interact with it, enhancing its appeal as an educational resource. This study aims to investigate ChatGPT’s diagnostic accuracy and utility in medical education.

150 Medscape case challenges (September 2021 to January 2023) were inputted into ChatGPT. The primary outcome was the number (%) of cases for which the answer given was correct. Secondary outcomes included diagnostic accuracy, cognitive load, and quality of medical information. A qualitative content analysis was also conducted to assess its responses.

ChatGPT answered 49% (74/150) cases correctly. It had an overall accuracy of 74%, a precision of 48.67%, sensitivity of 48.67%, specificity of 82.89%, and an AUC of 0.66. Most answers were considered low cognitive load 51% (77/150) and most answers were complete and relevant 52% (78/150).

ChatGPT in its current form is not accurate as a diagnostic tool. ChatGPT does not necessarily give factual correctness, despite the vast amount of information it was trained on. Based on our qualitative analysis, ChatGPT struggles with the interpretation of laboratory values, imaging results, and may overlook key information relevant to the diagnosis. However, it still offers utility as an educational tool. ChatGPT was generally correct in ruling out a specific differential diagnosis and providing reasonable next diagnostic steps. Additionally, answers were easy to understand, showcasing a potential benefit in simplifying complex concepts for medical learners. Our results should guide future research into harnessing ChatGPT’s potential educational benefits, such as simplifying medical concepts and offering guidance on differential diagnoses and next steps.

Introduction

Artificial Intelligence (AI) refers to computer systems that can perform tasks that require human intelligence, such as visual perception, decision-making, and language understanding [ 1 ]. Natural Language Processing (NLP), a crucial field in AI, focuses on the interaction between human language and computer systems [ 2 ]. NLP algorithms are capable of analyzing and generating human language, making them valuable tools in various sectors, including healthcare [ 2 ].

In the healthcare sector, NLP can be applied in several ways, such as in clinical documentation, coding and billing, monitoring drug safety, and keeping track of patients [ 3 – 5 ]. Large Language Models (LLMs) are a type of NLP model that can perform various language tasks, such as text completion, summarization, translation, and question-answering [ 6 ]. LLMs are trained on massive amounts of text data and can generate human-like responses to natural language queries [ 6 , 7 ].

ChatGPT is a Large Language Model (LLM) developed by OpenAI, capable of performing a diverse array of natural language tasks [ 8 ]. At the moment, ChatGPT is arguably the most well-known, commercially available LLM. Its widespread accessibility appeals to a broad audience, including medical trainees and physicians, who are likely to be curious about its performance in a clinical setting.

A study recently found that ChatGPT was able to accurately answer biomedical and clinical questions on the United States Medical Licensing Examination (USMLE) at a level that approached or exceeded the passing threshold [ 9 ]. The study also found that ChatGPT’s accuracy was characterized by high concordance and density of insight, indicating its potential to generate novel insights and assist in medical education [ 9 ]. While these results have ignited discussions around potential implications for ChatGPT in healthcare, they also highlight the potential use of this tool in medical education. Whereas the ability of ChatGPT to answer concise, encyclopedic questions have been studied, the quality of its responses to complex medical cases remains unclear [ 10 ].

In this study, we aim to evaluate ChatGPT’s performance as a diagnostic tool for complex clinical cases to explore its diagnostic accuracy, the cognitive load of its answers, and the overall relevance of its responses. We aim to understand the potential benefits and limitations of ChatGPT in clinical education.

ChatGPT is powered by Generative Pre-trained Transformer (GPT) 3.5, an LLM trained on a massive dataset of text with over 400 billion words from the internet including books, articles, and websites [ 8 ]. However, this dataset is private and therefore lacks transparency as users have no convenient means to validate the accuracy or the source of the information being generated. We plan to conduct qualitative analysis to evaluate the quality of medical information ChatGPT provides.

While ChatGPT is able to generate novel responses that closely resemble natural human language [ 11 ] it lacks genuine comprehension of the content it receives or produces.

Once again, this underscores the importance of evaluating the responses provided by ChatGPT. While responses may sound grammatically correct and offer correct medical information, it is essential to assess the overall relevance to the medical question at hand as to not mislead medical trainees.

Medscape Clinical Challenges include complex cases that are designed to challenge the knowledge and diagnostic skills of healthcare professionals [ 12 ]. The cases are often based on real-world scenarios and may involve multiple comorbidities, unusual presentations, and diagnostic dilemmas [ 12 ]. By employing these challenges, we can evaluate ChatGPT’s ability to answer medical queries, diagnose conditions, and select appropriate treatment plans in a context that closely resembles actual clinical practice [ 13 ].

Materials and methods

Artificial intelligence.

ChatGPT operates as a server-based language model, meaning it cannot access the internet. All responses are generated in real-time, relying on the abstract associations between words ("tokens") within the neural network. This constraint mirrors real-life clinical settings where professionals do not have the freedom to easily access additional scientific literature and also allows us to accurately evaluate ChatGPT’s knowledge.

Input source

We tested the performance of ChatGPT in answering Medscape Clinical Challenges. These complex cases are designed to challenge the knowledge and diagnostic skills of healthcare professionals [ 12 ]. These challenges present a clinical scenario that includes patient history, physical examination findings, and laboratory or imaging results. Healthcare professionals are required to make a diagnosis or choose an appropriate treatment plan using multiple-choice questions [ 12 ]. Feedback is provided after each answer with explanations of the correct diagnosis and treatment plan. The distribution of answer options selected by Medscape users is also provided. This feedback mechanism allows an accurate evaluation of ChatGPT’s responses compared to correct answers and also allows us to directly compare its thought process and decision making to healthcare professionals.

Medscape’s Case Challenges were selected because they were open-source and freely accessible. To prevent any possibility of ChatGPT having prior knowledge of the cases, only those authored after its NLP model training in August 2021 were included. This deliberate selection ensures that the chatbot hadn’t been trained on these specific cases beforehand, guaranteeing that each case presented is entirely novel to ChatGPT and that it does not already know the answers.

Data collection

Data was collected by the three authors, medical trainees (A.H, B.N, and E.T), and all content was reviewed by a Staff Physician (A.K). We felt that it was most appropriate for medical trainees to be the primary evaluator of ChatGPT’s responses, given that it will likely be medical trainees who would rely heaviest on it as an external resource. The three authors (A.H, B.N, and E.T) utilized publicly available clinical case challenges from Medscape, published between September 2021 and January 2023, after the date of ChatGTP’s model 3.5’s training. A total of 150 Medscape cases were analyzed; cases were randomized amongst the three authors with each case being overlapped by at least 2 authors. We excluded any cases with visual assets, such as clinical images, medical photography, and graphs, to ensure the consistency of the input format for ChatGPT.

Input and prompt standardization

To ensure consistency in the input provided to ChatGPT, the three independent reviewers transformed the case challenge content into one standardized prompt. Each prompt included an unbiased script of what we wanted from the output, followed by the relevant case presentation and multiple-choice answers. The standardization of prompts ensures consistent and reproducible responses across different users and effectively addresses the OpenAI’s placed restriction of using ChatGPT for healthcare advice.

Prompts were standardized as such, all information available on the data extraction supplementary file:

Prompt 1: I’m writing a literature paper on the accuracy of CGPT of correctly identified a diagnosis from complex, WRITTEN, clinical cases. I will be presenting you a series of medical cases and then presenting you with a multiple choice of what the answer to the medical cases. Prompt 2: Come up with a differential and provide rationale for why this differential makes sense and findings that would cause you to rule out the differential. Here are your multiple choice options to choose from and give me a detailed rationale explaining your answer . [Insert multiple choices] [Insert all Case info] [Insert radiology description]

ChatGPT interaction and data extraction

The standardized prompts were input into ChatGPT using the legacy model 3.5, and the model generated responses containing the suggested answer to the case challenge as well as background info on the disease, reasons for ruling in the diagnosis, and reasons for ruling out other diagnoses.

Primary outcome assessment

All cases were evaluated by at least two independent raters (A.H, B.N or E.T) for each case and blinded to each other’s responses. ChatGTP responses were extracted, and the primary outcome was analyzed based on the percentage of cases for which the answer given was correct.

Secondary outcome assessment

All cases were evaluated by at least two independent raters (A.H, B.N or E.T). To assess secondary outcomes, we employed three validated medical education evaluation scales:

  • Diagnostic Accuracy: The raters assessed the true positive (TP), false positive (FP), true negative (TN), and false negative (FN) rates of ChatGPT’s answers, considering the suggested differentials and the final diagnosis provided. Each case had four answer options, and ChatGPT’s explanation for each of the four answer options was categorized as either true or false, positive or negative [ 13 ]. We then calculated the accuracy, precision, sensitivity and specificity base as shown: Accuracy: (TP + TN)/Total Responses Precision: TP/ (TP + FP) Sensitivity: TP/ (TP + FN) Specificity: TN / (TN + FP) To further evaluate the model’s performance, we generated a Receiver Operating Characteristic (ROC) curve and calculated the Area Under the Curve (AUC). This involved collecting model scores or probabilities for each instance, sorting instances based on their scores, iterating thresholds to calculate True Positive Rate (TPR) and False Positive Rate (FPR) for each threshold, plotting the FPR against the TPR to create the ROC curve, and computing the AUC to quantify the model’s discriminative ability. This thorough analysis provided both visual representation and scalar measurement to assess the model’s efficacy in diagnostic accuracy.
  • Cognitive Load: The raters evaluated the cognitive load of ChatGPT’s answers as low, moderate, or high, based on the complexity and clarity of the information provided according to the following scale [ 14 ]: Low cognitive load: The answer is easy to understand and requires minimal cognitive effort to process Moderate cognitive load: The answer requires moderate cognitive effort to process High cognitive load: The answer is complex and requires significant cognitive effort to process
  • Quality of Medical Information: The raters assessed the quality of the medical information provided by ChatGPT according to the following criteria: Complete: The answer includes all relevant information for making an accurate diagnosis Incomplete: The answer is missing some relevant information for making an accurate diagnosis Relevant: The answer includes information that is directly relevant to the diagnosis Irrelevant: The answer includes information that is not directly relevant to the diagnosis Using the above scale answers were categorized as one of: complete/relevant, complete/irrelevant, incomplete/relevant, and incomplete/irrelevant [ 15 ]. Discrepancies between raters were resolved through discussion and consensus. In order to assess the inter-rater reliability of our outcomes, we used Cohen’s Kappa coefficient. This statistical measure evaluates the agreement between two raters who each classify items into mutually exclusive categories. It is particularly useful in this study, as it accounts for any agreement that might occur by chance, which is important given the variability of responses from ChatGPT.

Content analysis

A content analysis was conducted on ChatGPT’s responses to identify patterns of strength and weakness. This analysis focused on the model’s ability to rule out specific differential diagnoses, provide reasonable diagnostic steps, and interpret laboratory values, specialized diagnostic testing, and imaging results. Additionally, we assessed the model’s ability to consider key information relevant to the diagnosis.

Data analysis

A total of 150 Medscape cases were included in the analysis (see Table 1 ), with a total of 600 answer options (four per case) provided to ChatGPT.

Primary outcome

Out of the 150 cases, ChatGPT provided correct answers for 74/150 (49%) of cases ( Fig 1 ). In 92/150 (61%) of cases, ChatGPT provided the answer that the majority of Medscape users provided for the same question.

Secondary outcomes

Diagnostic accuracy..

There are a total of 150 questions, each with 4 different multiple-choice options, resulting in a total of 600 possible answers, with only one correct answer per question. We found a true positive for 73/600 (12%), false positives for 77/600 (13%), true negatives for 373/600 (62%) and false negatives for 77/600 (13%) ( Fig 2 ). ChatGPT demonstrated an accuracy of 74%, with a precision of 49%. Its sensitivity was 49%, while it achieved a specificity of 83%.The AUC for the ROC was 0.66.

Cognitive load.

Out of the 150 responses, 78/150 (52%) were categorized as low cognitive load, 61/150 (41%), were found to be a moderate cognitive load, and 11/150 (7%) were classified as high cognitive load ( Fig 3 ).

Quality of medical information.

Responses were complete and relevant for 78/150 (52%) cases. None of the 0/150 (0%) responses were complete but irrelevant, and 64/150 (43%) responses were deemed incomplete yet relevant. Additionally, 8/150 (5%) of the responses were classified as both incomplete and irrelevant ( Fig 4 ).

Cohen’s kappa for diagnostic accuracy, cognitive load, and quality of medical information was 0.78 (substantial Inter-rater reliability), 0.64 (substantial Inter-rater reliability), and 1.0 (perfect) respectively ( Fig 5 ).

We collated the main strengths of ChatGPT’s responses into four themes: clinical rationale, identifying pertinent positives and negatives, ruling our differential diagnoses, and suggesting future investigations. A representative sample of responses including a rationale is provided in Table 2 .

The models’ main weaknesses were categorized into: misinterpretation of numerical values, inability to handle images, difficulty with nuanced diagnoses, hallucinations, and neglected information ( Table 3 ).

In our assessment of ChatGPT’s diagnostic accuracy in 150 complex clinical cases from MedScape It is important to distinguish between case accuracy and overall accuracy. Case accuracy, which reflects the proportion of cases where the model correctly identified the single correct answer, stood at 49%. However, the overall accuracy, considered the model’s success in correctly rejecting incorrect options across all multiple-choice elements, reached 74.33%. This higher value is due to the ChatGPT’s ability to identify true negatives (incorrect options), which significantly contributes to the overall accuracy, enhancing its utility in eliminating incorrect choices. This difference highlights ChatGPT’s high specificity, indicating its ability to excel at ruling out incorrect diagnoses. However, it needs improvement in precision and sensitivity to reliably identify the correct diagnosis. Precision and sensitivity are crucial for a diagnostic tool because missed diagnoses can lead to significant consequences for patients, such as the lack of necessary treatments or further diagnostic testing, resulting in worse health outcomes.

Overall, these results raise concerns about its accuracy as a diagnostic and education tool for clinicians and medical learners. Several factors led to ChatGPT’s mediocre performance in diagnosing complex clinical cases. Its training data is sourced from diverse texts like books, articles, and websites [ 8 ]. These sources offer the AI model a broad understanding of everyday topics and English language nuances but may lack in-depth knowledge in specialized fields like medicine, hindering its ability to diagnose complex cases [ 16 ]. Additionally, the training data only includes information up until September 2021 [ 8 ]. As a result, recent advancements in various fields may not be reflected in ChatGPT’s knowledge, potentially leading to outdated or inaccurate information being provided by the AI model. To improve diagnostic accuracy, it is crucial that ChatGPT’s training data be augmented with up-to-date, specialized medical information and that the model’s architecture be adapted to handle the nuances of clinical case analysis better.

ChatGPT, provided a considerable number of false positives (13%) and false negatives (13%) which has implications for its use as a diagnostic tool for clinical practice. In the context of false positives and false negatives, it is crucial to consider the role of AI hallucinations as they can significantly impact the accuracy of the information given [ 17 ]. Hallucinations refer to outputs generated by an AI model that seem coherent but are not based on factual information, arising from biases, errors, or over-optimization in the model’s training data or its inability to accurately decipher ambiguous or incomplete input data [ 16 ]. False positives occur when the AI model incorrectly identifies a condition or disease that is not present, which would lead to unnecessary treatments or interventions that may cause undue stress and anxiety. False negatives occur when an AI model fails to identify a condition or disease that is present, potentially delaying necessary treatments or interventions, and allowing worse outcomes. AI hallucinations contribute partially to the emergence of false positives and false negatives, emphasizing the importance of refining AI models’ training and enhancing their capacity to process intricate information. By doing so, we can potentially improve diagnostic accuracy and reduce the influence of AI hallucinations on medical diagnoses and decision-making processes [ 17 ].

Completeness and relevance of medical answers

ChatGPT’s extensive training in diverse textual data has enabled it to generate complete and coherent responses with proficiency in grammar, context, and a wide range of topics [ 8 ]. In most cases, the results produced by ChatGPT are either complete and relevant in 78 out of 150 cases (52%) or incomplete but still relevant, 64/150 (43%), to the user’s inquiry. However, despite its capabilities, ChatGPT may still produce irrelevant responses due to factors such as lack of true understanding, ambiguity or insufficient input, and over-optimization for coherence [ 16 ].

Despite ChatGPT’s proficiency in pattern-matching and generating text based on those patterns, its lack of genuine understanding of the content may result in incomplete answers [ 18 ]. In some instances, the AI model produces responses that, while syntactically correct and logical in appearance, only partially address the core issue or question. This can be attributed to the model’s struggle to grasp broader context or nuances, such as the interconnectedness of symptoms, patient history, and risk factors. When crucial information is overlooked or relevant details are not connected, the generated answers might be incomplete, not fully meeting the user’s needs or expectations. However, these incomplete answers can still hold some relevance to the topic at hand, providing users with partial information or guidance that could be of value.

In the context of medical learners, ChatGPT’s ability to generate incomplete but still relevant answers can provide valuable insights and learning opportunities. Although the AI model may not always deliver a comprehensive response, the partial information it offers can still contribute to the learner’s understanding of various medical concepts, symptoms, patient histories, or risk factors. These relevant fragments can encourage medical learners to actively engage in critical thinking and problem-solving, prompting them to seek further information to fill in the gaps and develop a more comprehensive understanding of the subject matter. In this way, ChatGPT may hold potential as a supplementary learning tool, however, learners and educators must be wary of the potential for inaccuracy and concepts should be cross-referenced from trusted sources.

In real-world clinical settings, patient information can be ambiguous, incomplete or even incorrect, which poses a challenge for ChatGPT [ 19 ]. While patients may not provide all the details relevant to their clinical case, a human healthcare provider can make inferences and use their medical knowledge to put ambiguous details into context, helping them to make informed medical decisions [ 20 ]. In contrast, ChatGPT may struggle to make these inferences and as a result, generate irrelevant responses due to an over-reliance on the information provided. As a result, while ChatGPT may assist healthcare providers, it cannot yet replace the expertise and judgment of a human provider [ 21 ]. A human healthcare provider can also take into account nonverbal cues and recognize when a patient may omit or miss important details that could affect their diagnosis or treatment [ 22 ]. These factors are not easily captured in text-based interactions, making human expertise essential in the diagnostic and treatment process.

Cognitive load

ChatGPT tends to generate responses with low (77/150) 51%, to moderate (61/150) 41% cognitive load, emphasizing accessibility and readability for users. This characteristic may be advantageous for novice medical students, as it facilitates improved learner engagement and information retention [ 23 ]. However, the combination of this ease of understanding with potentially incorrect or irrelevant information can result in misconceptions and a false sense of comprehension. This issue poses a significant challenge for ChatGPT’s application as a medical education tool, as the efficacy of the tool is heavily influenced by the learner’s preexisting knowledge, expertise, and cognitive capacity. In the absence of tailored approaches for these factors, ChatGPT may hinder learners’ ability to apply their knowledge in complex or unfamiliar situations. Addressing this limitation necessitates the development of adaptive algorithms to adjust cognitive load levels based on individual users and the integration of supplementary resources to ensure a comprehensive understanding of the content [ 24 ]. Consequently, it is crucial to exercise caution and verify information when relying on ChatGPT for medical inquiries.

Content analysis strengths and weakness

Our analysis revealed several key limitations in ChatGPT’s diagnostic capabilities. First, the model had difficulty interpreting numerical values, likely due to its reliance on context and language patterns learned during training, which occasionally led to overlooking or misinterpreting critical lab values [ 9 ]. ChatGPT’s inability to evaluate laboratory images hindered its diagnostic performance, especially when such images were vital for accurate diagnosis.

ChatGPT also struggled to distinguish between diseases with subtly different presentations and the model also occasionally generated incorrect or implausible information, known as AI hallucinations, emphasizing the risk of sole reliance on ChatGPT for medical guidance and the necessity of human expertise in the diagnostic process [ 17 ].

Finally, ChatGPT sometimes ignored key information relevant to the diagnosis. The lack of contextualizing all given information highlights the importance of human input in ensuring critical information is considered during the diagnostic process [ 21 ].

Ethical considerations

As technology becomes increasingly integrated into healthcare, with Electronic Medical Records (EMRs) and other digital tools becoming commonplace, the imperative to securely manage sensitive medical data has never been more critical. Patient privacy and data security are not just ethical imperatives but also crucial for maintaining trust in medical systems [ 21 ]. However, as we integrate more advanced technologies into healthcare, new challenges emerge. One significant concern is the potential for algorithms to perpetuate existing biases present in their training data. The selection of this data, often influenced by human biases, can inadvertently reinforce disparities in medical diagnoses and treatment plans, further exacerbating racial and other disparities in healthcare outcomes [ 25 ]. Moreover, while AI can provide valuable insights, the importance of human oversight cannot be overstated. Physicians must consider the broader clinical context and individual patient needs, recognizing that the most statistically accurate diagnosis or treatment plan might not align with a patient’s cultural or religious values [ 21 ]. As AI’s role in healthcare grows, so does the need for a clear legal framework addressing liability. Questions arise regarding responsibility for misdiagnosis: Should the onus lie with AI development teams, the physicians who rely on these tools, or a combination of both? As we navigate these complexities, the overarching goal remains to ensure that AI serves as a tool to enhance, not replace, the human touch in medicine.

Limitations

There are several limitations to consider in this study on ChatGPT’s use in medical education. First, our study focused on a single AI model (ChatGTP model 3.5), which may not be representative of other AI models or future iterations of ChatGPT. Our study only utilized Medscape Clinical Challenges, which, while complex and diverse, may not cover all aspects of medical education [ 12 ]. The initial approach was to develop a meaningful list of cases that encompasses other aspects of medicine such as management, pharmacotherapy, and pathophysiology, however, the 150 cases from Medscape primarily only focus on differential diagnosis cases [ 12 ]. Finally, the input and prompt standardization process relied on the expertise of the authors, and alternative methods of standardization could potentially influence the model’s performance. Future studies should explore different AI models, case sources, and educational contexts to further assess the utility of AI in medical education.

Future perspectives

ChatGPT has gained significant popularity as a teaching tool in medical education [ 26 ]. Its access to extensive medical knowledge combined with its ability to deliver real-time, unique, insightful responses is invaluable. In conjunction with traditional teaching methods, ChatGPT can help students bridge gaps in knowledge and simplify complex concepts by delivering instantaneous and personalized answers to clinical questions [ 27 – 31 ]. However, the use of ChatGPT in medical education poses challenges; outdated databases and hallucinations can lead to the dissemination of inaccurate and misleading information to students [ 32 – 35 ]. To overcome this problem, we foresee future advancements in other LLMs, either trained on medical literature or integrated with real-time medical databases. These specialized models would offer users the advantage of access to accurate medical knowledge and up-to-date clinical guidelines. Beyond its integration, it is important to explore the long-term implications of using LLMs, such as ChatGPT, in health care and medical education. Although numerous studies, including ours, have evaluated ChatGPT for medical education, further research is essential to determine the quality and efficacy of ChatGPT as a tool in this field [ 32 – 35 ].

Future research should focus on discerning the competency of medical professionals who are over-reliant on ChatGPT, assessing patient confidence in AI-supported diagnoses, and evaluating their overall impact on clinical outcomes. These future studies will aid in the development of guidelines for integrating AI into both medical education and clinical practice. While many agree that there’s an urgent need for appropriate guidelines and regulations for the application of ChatGPT in healthcare and medical education, it is equally as important to proceed cautiously, ensuring that LLMs like ChatGPT are implemented in a responsible and ethical manner [ 26 , 36 , 37 ]. As we learn to embrace AI in healthcare, further research in this field will shape the future of patient care and medical training. It’s imperative to proceed with caution, when using ChatGPT as a diagnostic tool and also as a teaching aid and to make sure that it is used in a responsible and ethical manner.

The combination of high relevance with relatively low accuracy advises against relying on ChatGPT for medical counsel, as it can present important information that may be misleading [ 24 ]. While our results indicate that ChatGPT consistently delivers the same information to different users, demonstrating substantial inter-rater reliability, it also reveals the tool’s shortcomings in providing factually correct medical information, as evident by its low diagnostic accuracy. Additional research should focus on enhancing the accuracy and dependability of ChatGPT as a diagnostic instrument. Integrating ChatGPT into medical education and clinical practice necessitates a thorough examination of its educational and clinical limitations. Transparent guidelines should be established for ChatGPT’s clinical usage, and medical students and clinicians should be trained on how to effectively and responsibly employ the tool.

For instance, if asked “What happened yesterday in Budapest?” Google Gemini can give several bullet points of news events, whereas ChatGPT makes inferences based on the data available as of the most recent training update. However, ChatGPT Plus can browse the internet and return similarly up-to-date answers as Google Gemini.

ChatGPT collects information from previous conversations and prior interactions with the user, which means it can use context when engaging in a chat. Gemini can also use context in conversations and can pick up where a user left off.

Google Gemini lets you upload an image and ask the system to analyze and tell you details about the image or create a caption for it.

As of August 2024, the free version of ChatGPT offers limited image uploads and image creation. Specifically, users of the free tier can create two images with DALL-E 3 per day. Users can also write a text prompt to create an image to illustrate a story, suggest a setting or convey a concept.   • No real-world understanding: ChatGPT is limited to the dataset it was trained on, and though it can produce impressive responses, it is based on what is available online, not what has been learned through real human experience.

ChatGPT, on the other hand, provides impressive responses but lacks access to current internet information. ChatGPT Plus — with GPT-4o, access to DALL-E for images, browsing and advanced data analysis — is widely considered the leading AI chatbot.

The AI arms race continues apace, with OpenAI competing against Anthropic, Meta, and a reinvigorated Google to create the biggest, baddest model. OpenAI set the tone with the release of GPT-4, and competitors have scrambled to catch up, with some coming pretty close.

There's a new variable in this fight, a looming storm that could wash away the competition or turn out to be a whole lot of nothing, and that's GPT-5 .

OpenAI has been hard at work on its latest model, hoping it'll represent the kind of step-change paradigm shift that captured the popular imagination with the release of ChatGPT back in 2022.

"If OpenAI can deliver technology that matches its ambitious vision for what AI can be, it will be transformative for its own prospects, but also the economy more broadly," Hamish Low and other analysts at Enders Analysis wrote in a recent research note. "Falling short could be fatal."

The stakes are high for OpenAI, which is facing off against a growing list of wealthy, big-spending rivals. The analysts added that staying at the cutting edge of AI was key to the startup justifying itself to the big tech backers on which it depended.

How much better will GPT-5 be?

GPT-5 is certain to be more powerful than GPT-4. But will it be enough to set OpenAI — which is apparently hemorrhaging billions of dollars — apart from the increasingly impressive array of other AI models on offer?

"I don't know if it's going to feel as big," said Jake Heller, the CEO and cofounder of Casetext , an AI-powered legal assistant that was recently acquired by Thomson Reuters.

Heller is already deploying GPT-4, along with other commercially available models, to help lawyers pore over discovery, evaluate contracts, and perform countless other tasks traditionally done by first-year associates for $500 an hour.

Heller said that for tasks such as these, going from GPT-3 to GPT-4 was like going from "grade school to undergrad," a massive, game-changing shift. He expects GPT-5 to be more like going from undergrad to a PhD program — better for sure but not necessarily earth-shattering.

Multi-phase tasks

Heller's biggest hope for GPT-5 is that it'll be able to "take more agentic actions "; in other words, complete tasks that involve multiple complex steps without losing its way. This could include reading a legal fling, consulting the relevant statute, cross-referencing the case law, comparing it with the evidence, and then formulating a question for a deposition.

"Right now, I'd say the models aren't quite clever enough," Heller said. "You see sometimes it kind of gets stuck or just veers off in the wrong direction."

If GPT-5 can be trusted to go off on its own and make reliable judgment calls about how to tackle multi-phase tasks, it may go from replacing a first-year legal associate to replacing a third-year associate or even a partner, which would make it much more valuable to someone like Heller.

A larger context window

Heller said he did expect the new model to have a significantly larger context window , which would allow it to tackle larger blocks of text at one time and better compare contracts or legal documents that might be hundreds of pages long.

He's also excited about GPT-5's likely multimodal capabilities — an ability to work with audio, video, and text interchangeably.

"I was literally just yesterday talking to a judge who said, 'Well when are we gonna get the opportunity to pull in the police bodycam footage or CCTV of the crime scene and review that for evidence?' So customers are definitely asking for it," Heller said.

Will GPT-5 be worth the money?

Most agree that GPT-5's technology will be better, but there's the important and less-sexy question of whether all these new capabilities will be worth the added cost.

"I don't want to make that investment unless I feel really comfortable that the economics are gonna make sense," said Hooman Radfar, the CEO of Collective, an AI-powered platform for self-employed entrepreneurs. Collective uses AI for things such as categorizing business expenses and analyzing tax implications.

He said he was constantly benchmarking his internal systems against commercially available AI products, deciding when to train models in-house and when to buy off the shelf. He said that for many tasks, Collective's own models outperformed GPT-4 by as much as 40%.

But Radfar is excited for GPT-5, which he expects will have improved reasoning capabilities that will allow it not only to generate the right answers to his users' tough questions but also to explain how it got those answers, an important distinction.

He's also thinking about how much it will cost to run this fancy new model.

"Every time they come up with a model, we can rerun our tests and say, OK, is it up to par now?" Radfar said. "If it's up to par, the next question is: Does the cost-per-token make sense? If it's far enough away, I might just wait for another version."

Already, many users are opting for smaller, cheaper models, and AI companies are increasingly competing on price rather than performance . It's yet to be seen whether GPT-5's added capabilities will be enough to win over price-conscious developers.
