COVID-19 spread and vaccines

NIH researchers continued to make scientific breakthroughs to help control the COVID-19 pandemic. Studies of spread suggested strategies for controlling infections . Research also revealed who was most at risk of becoming severely ill from COVID-19: nearly two-thirds of COVID-19 hospitalizations in the U.S. were due to obesity, diabetes, hypertension, and heart failure . Wide-spread vaccine rollouts slowed the spread of SARS-CoV-2, the virus that causes COVID-19. The Moderna COVID-19 vaccine, developed with NIH, proved to be 94% effective against symptomatic COVID-19 . Six months later, people who had been vaccinated still showed signs of immunity . COVID-19 vaccines saved an estimated 140,000 lives through May 2021, and hundreds of thousands more have been saved since. As SARS-CoV-2 mutated and new variants became common, scientists conducted studies on booster doses of vaccine. These suggested that COVID-19 boosters not only lengthen immunity but help broaden and strengthen the immune response to protect against a wide variety of variants.

Drug delays type 1 diabetes onset

In type 1 diabetes, the immune system’s T cells attack the insulin-producing beta cells in the pancreas. Those affected need insulin treatment to survive. In a clinical trial of people at risk of developing type 1 diabetes (with a median age of 13), the drug teplizumab delayed disease onset and improved insulin production. The findings support the use of the drug for delaying or preventing type 1 diabetes.

Gene therapy effective in human trials

Advances in gene therapy are showing promise for people with life-threatening conditions. Using gene therapy, researchers were able to introduce a healthy copy of a disease-causing gene and restore immune system function in children with a rare genetic disorder called severe combined immunodeficiency, or SCID. In another study, researchers safely delivered gene therapy to the brain to treat a debilitating neurological disease for which there are no effective treatments.

Low-fat diet compared to low-carb diet

There has been a great deal of debate over what aspects of our diets affect weight control. A carefully controlled study found that people ate fewer calories per day and lost more weight on a plant-based, low-fat diet compared to an animal-based, low-carb diet. The findings reveal how restricting dietary carbohydrates or fats may impact health.

The effects of childhood malnutrition can cause lifelong health problems. A common blood test for kidney function measures a protein called creatinine. But Black Americans generally have higher amounts of creatinine. As a result, doctors take race into account when testing for kidney disease. A study showed that measuring levels of another protein called cystatin C can accurately estimate kidney function without needing to take race into account. A race-blind method for estimating kidney function could more effectively identify chronic kidney disease.

Malaria vaccines provide strong and lasting immunity

Malaria is caused by parasites transmitted by infected mosquitoes. Researchers developed a vaccine approach that uses live sporozoites, the infectious form of the malaria parasite, along with a drug that kills the parasite. In a small trial, the approach led to broad, long-lasting protection against malaria. The strategy is now being tested in real-world conditions in a Phase 2 clinical trial in Mali.

11 global health issues to watch in 2023, according to IHME experts

As the year 2022 winds down, what is next on the horizon for global health? We turned to our IHME experts for their takes on the most critical health issues to watch in 2023. Entering our fourth year grappling with COVID-19, most of our experts pointed to issues that were impacted in some way by the pandemic, like long COVID and mental health. They also offered potential interventions to address the threats. 

The faculty members and research scientists who shared their insights are professor Mohsen Naghavi , assistant professor Hwme Kyu , assistant professor Angela Micah , affiliate professor Michael Brauer , affiliate assistant professor Alize Ferrari , lead research scientist Liane Ong , lead research scientist Sarah Wulf Hanson, postdoctoral scholar Christian Razo, postdoctoral scholar Ewerton Cousin, and researcher Emma Nichols. Their comments have been lightly edited for clarity.

1. Long COVID

“Long COVID is absolutely a health issue to watch in 2023. The health impact of long COVID often disrupts a person’s ability to engage with school, work, or relationships for months at a time.   “People with long COVID need diagnostic and proper rehabilitation support from primary care physicians. We desperately need more research to find effective treatments as well as preventive measures to reduce the risk of developing long COVID.” — Sarah Wulf Hanson, lead research scientist of the non-fatal and risk quality enhancement team and lead author of the JAMA paper on long COVID     

2. Mental health

“Mental disorders are a leading cause of disability worldwide, with no evidence of a decrease in this burden since 1990. The impact of the COVID-19 pandemic, war, and violence on mental health remains a priority, specifically understanding how these have impacted the prevalence and burden of mental disorders in 2022 onward and how countries should be adapting their mental health response accordingly. 

“Currently in the GBD study, we investigate childhood sexual abuse, intimate partner violence, and bullying victimization as risk factors for mental disorders. Going forward, we need a better understanding of the other risk factors for mental disorders, how these vary across different populations, and how to offer the best opportunities for prevention at the population level.” — Alize Ferrari, affiliate assistant professor and team lead for estimating the burden of mental disorders     

3. Impact of climate change

“Climate change is already affecting the health of millions of people all over the world, and more importantly, climate change will worsen throughout this century. People are experiencing both the direct effects of extreme heat that we measure in the GBD and a myriad of indirect effects. Flooding can force people from their homes and affect their mental health, droughts and storms can impact food security and water availability, and wildfire smoke episodes can increase air pollution. As we know from the pandemic, preparedness is key, and we are far from prepared for the health impacts of a warmer climate.

“Most of the emphasis to date on climate change – and rightly so – has been on what we call mitigation: reducing the emissions that lead to global warming. Yet to date these efforts have been far too modest. We are now at a point where climate change is clearly with us, and much more attention needs to be put on minimizing the impacts on global health through adaptation or enhancing resilience. 

“One aspect of this is improving overall health and enhancing socioeconomic development because we know that those who are more vulnerable will suffer the most. In addition, there are technological solutions that can support adaptation , such as the use of drought-resistant crops, increasing vegetation in cities to reduce the urban heat island effect, or repurposing land use to adapt to rising sea levels. 

“Air pollution is one of the leading global risk factors that we evaluate in the GBD – currently responsible for about 8% of all global mortality – yet it is a problem with known solutions. Increasing the speed at which we address air pollution will save lives today. Those solutions will move the world closer to the net-zero carbon emissions goals that we need to ultimately address the causes of climate change.” — Michael Brauer, affiliate professor and team lead for estimating the burden of environmental, occupational, and dietary risk factors     

4. Cardiovascular disease

“Cardiovascular diseases such as ischemic heart disease and stroke are the leading causes of death globally, accounting for 28% of total deaths in 2021. Additionally, cardiovascular diseases substantially contribute to health loss and the economic burden on health care systems. Most cardiovascular diseases can be prevented by addressing modifiable cardiovascular risk factors such as high blood pressure, high cholesterol, obesity, dietary risks, smoking, and air pollution.” — Christian Razo, postdoctoral scholar on the team estimating cardiovascular disease burden and lead author of a Burden of Proof study on the effects of elevated systolic blood pressure on ischemic heart disease     

5. Lower respiratory infections

“Lower respiratory infections (LRI), especially respiratory syncytial virus (RSV) and influenza, are health issues to watch in 2023.   “ We saw a general decline in influenza and RSV infections in 2020 due to COVID-19 mitigation measures such as mask use and social distancing. With the relaxation of these measures, many young children who haven’t been exposed to RSV in the past couple of years are being infected, resulting in RSV outbreaks . Countries have also experienced a surge in influenza across all ages.    “ Annual influenza vaccination provides an opportunity to reduce the LRI burden attributable to flu. There is no vaccine yet to prevent RSV, but promising vaccine trials are underway . ” — Hmwe Kyu, assistant professor and team lead for estimating the burden of HIV, TB, and select infectious diseases    “ After experiencing significant disruptions to health care systems worldwide due to the COVID-19 pandemic, the increases in respiratory infections and other communicable diseases have been added to the existing burden of chronic non-communicable diseases, creating a dual burden of disease exacerbated by social inequalities observed globally.” — Christian Razo     

6. Poverty’s role in health

“It seems that poverty is the mother of inequality in health. The unequal distribution of resources has expanded due to climate change and increasing violence. Low- and middle-income countries experience worse health outcomes than high-income countries: the life expectancy is 34 years lower, the under-5 mortality around 100 times higher, deaths due to interpersonal violence and suicide are 30 times higher, and deaths attributable to antimicrobial resistance (AMR) are 12 times higher. We must urgently address the impact of poverty on health, life, and death.” — Mohsen Naghavi, professor and team lead for causes of death, shocks, intermediate causes and estimating the burden of AMR

“Strengthening health systems globally remains a critical aspect of what is needed for resilient health systems. This will be particularly relevant as countries refocus their resources and attention after the acute phase of the COVID-19 pandemic.

“I think what is needed is a longer-term commitment from donors and governments – financial and human resources, governance structures, management, information systems – to ensure that interventions are set up for long-term sustainability and can deliver the outcomes that are aspired to across health systems.” — Angela Micah, assistant professor and co-lead of the development assistance for health resource tracking team

“Given the immense strain of COVID-19 on primary care and hospital systems the past two-and-a-half years, attention should be paid to building back up the health care system and enabling frontline workers to do their jobs effectively. 8. Diabetes

“Diabetes is the fourth main cause of DALYs in Latin America and the Caribbean, and among the top five causes, it is the only one that shows an increase in the age-standardized rate compared to 1990. The burden of diabetes in the Americas is large, increasing, heterogeneous, and expanding, especially in countries in Central Latin America and the Caribbean. 

“Population-based interventions such as taxes and incentives, more informative food labeling, improving the built environment to facilitate exercise, and greater advocacy to inform people of the risk diabetes poses, combined with expanded health education to combat diabetes risk factors, seem the best options. Policies aimed to help avoid weight gain and improve dietary quality are also paramount. 

“Another important aspect is improving the response of health systems in terms of access and quality care. These should include universal access to low-cost insulin and oral anti-diabetic medication to decrease avoidable deaths from acute complications. ARPA-H launches Exploration Topic to improve access to life-changing cellular therapies

ARPA-H launches new Exploration Topic with the aim of improving access to life-changing cellular therapies, including Autologous Cell Therapy (ACT)  

Today, the Advanced Research Projects Agency for Health (ARPA-H) launched its Decentralized Engineering of Cells Informed by Dynamic Evidence (DECIDE) Exploration Topic (ET). DECIDE-ET aims to advance new quality assurance technologies and accelerate the collection of evidence necessary to establish reimbursement pathways for rare disease therapies. If successful, ARPA-H believes the work in this arena would increase patients’ access to life-changing therapies from Academic Medical Centers (AMCs). 

AMCs have developed many life-changing therapies for adults and children with rare diseases, including those using autologous cell therapy, or ACT. ACT begins with harvesting the patient’s own cells or tissues, then processing them into a treatment specifically for that patient.  

However, before ACT can be more widely adopted, non-clinical barriers must be overcome to allow for decentralized and tunable-scale manufacturing.  Conventional large-batch production pathways are not cost-effective, and technological innovation and investment have not yet focused on tools and methods to robustly validate a therapy’s quality and consistency for small-batch production. As a result, the costs of manufacturing and securing regulatory approval for small-batch ACT production are high compared to anticipated revenues, making for an unsustainable business model.  

“Patients, especially children with rare diseases, urgently need greater access to ACT,” said ARPA-H Program Manager Darshak Sanghavi, MD , who leads DECIDE-ET. “Many academic institutions are not in a position to change the dynamics around these therapies. DECIDE-ET aims to help develop a more cost-effective manufacturing and quality assurance automation for smaller-scale ACT production.”  

ARPA-H will make the technologies and methods it develops through DECIDE-ET accessible to industry stakeholders. It will also work with federal agencies on relevant regulatory and reimbursement issues, that may also present formidable challenges to downstream development and adoption. 

DECIDE is a 24-month, three-stage Exploration Topic with a single Technical Area. The three stages include: (1) method development, (2) testing and prototyping, and (3) deployment and validation. For more information, view the draft DECIDE-ET on SAM.gov.  

ETs are fast-paced efforts that pursue topics strategically aligned with ARPA-H Mission Offices. They provide foundational proofs-of-concept to be used in future research. They also allow for a streamlined solicitation and acquisition approach.

After extensive work over two years, Tahatikonhsontóntie' – the Quebec Network Environment in Indigenous Health Research (Qc-NEIHR), with the collaboration of Unité de Soutien SSA Québec, the Department of Family Medicine of McGill University, and the Office of Joyce’s Principle—is making public their report on cultural safety in the context of Indigenous health research.

Cultural safety has emerged as a response to systemic racism that has been experienced by Indigenous Peoples in public services, and it has been expanded to research. This report, thought to be the first on this topic in Quebec, is a follow-up to a roundtable discussion on the topic that took place in Montréal between April 26 and 28, 2023, which brought together 45 people who shared an interest in transforming Indigenous health research environments.

Evaluation framework for Jacaranda Place - the Queensland Adolescent Extended Treatment Centre

Learn about our evaluation framework for Jacaranda Place, the Queensland adolescent extended treatment centre for young people requiring mental health care.

We engaged the Queensland Centre for Mental Health Research (QCMHR) in 2019 to develop and support the implementation of an evaluation framework for the new adolescent extended treatment centre (AETC), now known as Jacaranda Place.

Jacaranda Place provides specialist extended care and intensive treatment for young people with severe and complex mental health needs.

Development of the framework was undertaken through an expert advisory group established for this purpose. The group included senior mental health and education staff, and consumer and carer representatives.

The framework considers short, medium- and long-term objectives and includes a range of data instruments routinely collected for mental health services, to enable evaluation and improvement activities.

The final summary report sets out an endorsed evaluation framework. It was published in December 2019.

Survey on tuberous sclerosis complex finds fragmented care, lack of information, scarcity of support

Researchers call on policymakers, decision-makers, and "those who hold the purse strings" to listen to the evidence and take action to enable individuals with TSC to receive the internationally recommended coordinated care they require to live full lives.

Tuberous sclerosis complex (TSC) is a rare genetic disease (approximate 1:6,000 birth incidence) with a wide variability of physical and neuropsychiatric symptoms. Patients require lifelong care from multiple health care specialists.

But what is known of TSC in the Republic of Ireland? Up to now, very little, despite the fact that there are an estimated 600 patients living with the disease. Today, the findings of the first national clinical audit of baseline TSC care, of a group of adult and pediatric patients attending epilepsy services, carried out by Trinity College Dublin researchers, have been published in the Journal of Rare Diseases .

The paucity of information surrounding the illness, including the health care of this group, what care is being provided, if any, the lived experience of this group and the needs of the health care professionals who provide their care, led the Trinity team to undertake this research.

Researchers found care to be fragmented and lacking a formal TSC clinic or network to provide the internationally recommended TSC care for this multiorgan complex disease.

Coordination of the many investigations and consultations required to manage TSC frequently falls on the shoulders of the patients, families and caregivers to navigate their care and health care system.

Researchers say this is not conducive with the International or UK TSC recommendations, where lifelong, personalized, multidisciplinary care, by a coordinated team of TSC experts is recommended. Nor is it in line with the WHO guidance on safe systems of care to improve patient safety and reduce available harm.

Key findings

  • Care was shown to be fragmented with no centralized TSC care coordination as per the UK recommendations.
  • Numbers ( adult and children) attending epilepsy services were lower than expected. 135 patients were attended compared to approx. 400 patients that would have been expected to attend.
  • Many baseline investigations were completed especially in pediatric services. However, separate referrals were required to access services and treatments even within the same hospital, resulting in fragmented care.
  • All specialties required for the care of TSC patients were available in the ROI, e.g., neurology, nephrology and investigative technology such as MRI were available. However, long waiting times and no clear feedback systems resulted in fragmented uncoordinated care.

Key recommendations

The establishment of a TSC center of excellence in the ROI, with the recruitment of a TSC coordinator.

Coordinated care, ideally in a center of excellence, would facilitate health care professionals with TSC expertise to provide lifelong personalized care to TSC patients and support to their HCP colleagues. There would be an amplification in TSC awareness, increased patient safety and outcomes, improved patient and provider satisfaction and a reduction in health care costs.

Researchers note that an advanced nurse practitioner passionate about TSC has already begun to coordinate TSC patient care of those who are attending St James Hospital in Dublin. The team report that this has been lifechanging for these patients and the service needs to expand.

The research team highlighted that in recent years, important progress has been made in the management and control of TSC with the emergence of new pharmaceutical treatments. It is vital people receive an early diagnosis and the appropriate treatment.

Mary Vasseghi, Ph.D. candidate, School of Medicine Trinity College Dublin and FutureNeuro, the study lead author, said, "The understanding of the mechanism of TSC has changed and critically, improved the trajectory and outcomes for individuals living with TSC .

"The development of targeted pharmaceutical treatments and the establishment of International Clinical Recommendations for the management of this complex disease has greatly contributed to this progress.

"It is vital individuals receive an early diagnosis and ongoing appropriate surveillance and management by a coordinated team with TSC expertise- for life. We must provide coordinated integrated care for these patients. It is life-saving and life-changing.

As a parent of someone living with TSC and a new researcher in this field, I feel privileged to have this unique perspective."

Claire Behan, Ph.D. candidate, Academic Unit of Neurology, Trinity College Dublin and Registered Advanced Nurse Practitioner Epilepsy, Neurology Department, St. James's Hospital and co-author, said,

"Tuberous sclerosis is a rare genetic disorder that significantly impacts individuals and families in Ireland. Raising awareness through research like this is essential in supporting those affected. The backbone of this research is the network of advanced nurse practitioners in epilepsy across Ireland, highlighting the impact of interdisciplinary, collaborative research.

"Based on the findings from our initial audit, we began implementing changes at a local level. However, with these latest insights, it has become clear that these improvements should be extended and standardized at a national level.

"Policymakers, decision-makers, and those who hold the purse strings must listen to the evidence before them and take action. We need to empower parents to focus on their families and enable individuals with TSC to live their lives fully—free from the burden of navigating complex care systems.

"It's time to stand with our international colleagues in delivering a seamless, inclusive, and efficient service for people in Ireland with TSC, grounded in evidence and ultimately saving costs. The responsibility to act is now."
