In addition to knowing how to  interpret a chest X-ray (CXR), it’s also important to understand how to appropriately document the findings of a CXR in the notes. This guide provides a structured approach to documenting the findings of a CXR in a patient’s notes.

Documentation basics

Before we discuss how to document a CXR, we need to cover the basics that apply to all documentation in a patient’s notes. You can check out our detailed guide to writing in the notes  for more information.

What should I use to write with?

You need to use a pen with  black   ink , as this is the most legible if notes are photocopied.

Patient details

For every new sheet of paper your first task should be to document at least three key identifiers for the relevant patient:

• Date of birth
• Unique patient identifier
• Home address

If a patient label containing at least three identifiers is available, then this can be used instead of writing out the information manually.

Location details

You should indicate the patient’s current location on the continuation sheet:

Beginning your entry in the notes

At this point, you should already be holding a pen with black ink and you should have ensured the continuation sheet has at least three key patient identifiers at the top.

The next documentation steps include:

1.  Adding the date and time (in 24-hour format) of your entry.

2.  Writing your name and role as an underlined heading.

3.  Adding your entry in the notes below this heading (see the next section for details).

Documenting the CXR results in the notes

1. Document the time and date that the CXR was performed as this may be significantly different from the time you are documenting.

2. Write the indication for the CXR (e.g. “productive cough, shortness of breath and fever – possible community-acquired pneumonia” )

3. Document your interpretation of the CXR:

• Inspiration
• Carina and bronchi
• Hilar structures
• Lung fields
• Heart borders
• Position/shape
• Costophrenic angles
• Mediastinal contours
• Tubes/valves/devices

4. Document your overall impression of the CXR (e.g. “left lower lobe consolidation” ).

5. Document your plan based on the CXR findings.

Completing the entry in the notes

At the  end of your entry  to need to  include  the  following :

• Your full name
• Your grade/role (e.g. F2/Medical Registrar)
• Your signature
• Your professional registration number (e.g. GMC number)
• Your contact number (e.g. phone/bleep)

Lateral View Chest X-ray

Lateral view chest X-ray is an X-ray done from the side of the chest.  This is usually done together with a view from the front of the chest, also called a frontal view.  Lateral view of the chest is not always done, particularly when the chest X-ray is done portably or is not ordered by the clinical doctor.

Why are two views of the chest done?

A two view chest called a PA and lateral views is the most complete assesment of the chest using X-ray.  Two views are done because one view may not show all the abnormalities.  One view may show an abnormality that can be better clarified on the lateral view.   A lateral view also lets us localize abnormalities better to a specific anatomic site.

What does the lateral view of the chest show?

A lateral view of the chest can show more abnormalities like pneumonia, nodules and masses compared with just a frontal view of the chest.  Sometimes pneumonia can hide behind the heart or mediastinum and the lateral view will best show it.  The lateral view can also let us see a structure in two ways letting us come to a more accurate diagnosis.

Nodules and masses which can be cancerous are also sometimes seen only on the lateral view.  Nodules and masses may be seen by the spine, along the diaphragms and about the mediastinum on the lateral view.  Some nodules and masses of the lung are better seen on the lateral view.

An abnormality in the lung can better be localized on the lateral view.  We can tell on the lateral view if the abnormality is in the front or the back of the chest.  We can tell if an abnormality is in the mediastinum or hila versus the lung next to those structures.

A lateral view is also better to look at the bones of the spine and sternum.  Lateral view also shows parts of the lungs and heart to better advantage.  Therefore, a complete evaluation of the chest will include close inspection of both the frontal and lateral views of the chest.  There are blind spots on the frontal view alone.

Lateral view of the chest when done with the frontal view offers a more complete evaluation of the chest.   We can sometimes see abnormalities on the lateral view only like pneumonia or cancer.  The lateral view also lets us offer a more accurate diagnosis in some cases.  The lateral view of the chest also lets us say where the abnormality is in the chest.

The quality of the image can be assessed using the mnemonic PIER:

• position: is this a supine AP file? PA? Lateral?
• inspiration: count the posterior ribs. You should see 10 to 11 ribs with a good inspiratory effect
• exposure: well-exposed films have good lung detail and an outline of the spinal column
• rotation: the space between the medial clavicle and the margin of the adjacent vertebrae should be roughly equal to each other; look for indwelling lines or objects

Scan the bones for symmetry, fractures, osteoporosis, and lesions. Evaluate the soft tissues for foreign bodies, swelling, and subcutaneous air.

Evaluate the heart size: the heart should be <50% of the chest diameter on PA films and <60% on AP films. Check for the heart shape, calcifications, and prosthetic valves.

Check the hemidiaphragms for position (the right is commonly slightly higher than the left due to the liver) and shape (may be flattened bilaterally in chronic asthma or emphysema , or unilaterally in case of tension pneumothorax or foreign body aspiration ). Look below the diaphragm for free gas .

Pleural effusions may be large and obvious or small and subtle. Always check the costophrenic angles for sharpness ( blunted angles may indicate small effusions). Check the lateral film for small posterior effusions (more sensitive for small effusions).

Check lungs for infiltrates (interstitial vs. alveolar), masses, consolidation (+/-  air bronchograms ),  pneumothoraces , and vascular markings. Vessels should taper and should be almost invisible at the lung periphery.

Evaluate the major and minor fissures for thickening, fluid or change in position.

Check the position of foreign bodies e.g. ETT , NGT , pacemaker leads , central venous lines etc. Comment on previous surgery e.g. cholecystectomy clips, sternotomy wires.

Check aortic size and shape and the outlines of pulmonary vessels. The aortic knob should be clearly seen. The gastric bubble  should be seen clearly and not displaced.

Evaluate the hila for lymphadenopathy , calcifications, and masses. The left hilum is normally higher than the right. Check for widening of the mediastinum (which may indicate aortic dissection  in the appropriate clinical setting) and tracheal deviation (which may indicate a mass effect, e.g. from large goiter , or tension pneumothorax ). In children, be careful not to mistake the thymus for a mass!

In most cases, an impression is worthwhile as it not only forces you to synthesize all the findings together but acts as a double check.
