Clinical attachments are a useful way for international medical graduates to get a taste of the working lives of doctors in the NHS. You can observe a consultant in a relevant specialty without having the responsibility of patient care.

What is a clinical attachment?

A clinical attachment will help you prepare for working in the NHS.

It allows you to gain an overview of medical processes and systems in the NHS, by observing a consultant in a relevant speciality at work.

Attachments usually last between two and four months and cost from £400.

During the attachment, you are not given any responsibility and are not able to make clinical decisions or give clinical advice.

After a period observing the consultant, you may start to take on some clinical duties. This will be following a risk assessment, and at under the supervision of the consultant.

Duties are generally limited to:

  • observing consultations
  • participating in patient administration
  • taking patient histories
  • physical examinations
  • observing surgery.

Should I do a clinical attachment?

It is not a requirement for GMC registration or working in the NHS. However, you may find the experience usefully contributes towards showing that you meet the standards expected of doctors.

The experience you gain will be relevant to your GMC application for registration. It will ask for details of all your work experience from the last five years both medical and non-medical.

Experience shows that it is advisable to do a clinical attachment shortly before taking part two of the PLAB test or after you have passed it. This ensures you will get the most out of it.

The BMA does not support clinical attachments which charge and believes them to be exploitative, but understands that doctors may still wish to pursue them, and therefore present this information in order to help you make your decision.

Eligibility

You will need to meet the following conditions:

  • successful completion of a criminal records check
  • documents giving your proof of identity
  • if you are not a native English speaker, you will need an IELTS or OET certificate at level 7.5 or an equivalent level of English that you can demonstrate
  • occupational health clearance from the NHS trust
  • attendance at the doctors' induction day before the attachment commences, if applicable
  • satisfactory references from referees cited in the application form.

Generally, international medical graduates from outside the EEA need a standard visitor visa to come to the UK.

A standard visitor visa lasts for six months maximum, during which you can take the PLAB 2 test and complete the clinical attachment.

The current guidance Visit (publishing.service.gov.uk) includes information about applications for visas, permission to enter and permission to stay in the UK for visitors. Page 33 covers Graduates of medical, dental and nursing schools.

Finding a clinical attachment

There is no UK-wide tool for finding a clinical attachment.

You can search the internet for clinical attachments including NHS hospitals and post graduate deaneries in your search. 

Alternatively, you could try asking any friends or family working in an NHS hospital if they can introduce you to a consultant in your specialty.

  • Give details about internship:

Many international medical graduates don’t know much about what to expect once they arrive in the UK and start working in the NHS. In fact, for some, arriving on their health and care visa is the first time they ever set foot in the UK! While that’s not necessarily a bad thing, as the NHS provides a thorough induction period for IMGs, it’s still more desirable to know a little more about the process. That’s where clinical attachments come in handy.

What is a Clinical Attachment? 

A clinical attachment is an arrangement in which an IMG travels to the UK to observe an NHS consultant in a particular speciality. It usually takes place in a hospital. During the clinical attachment, the IMG holds no responsibility, particularly when it comes to patient care. Instead, they shadow the consultant, the primary purpose being to understand how the NHS medical system works.

Clinical attachments tend to last between two and six weeks (although that can vary). You will make no clinical decisions during this time, but you might take on some supervised clinical duties. That might include administration, performing a physical exam, or taking a patient’s history.

The Benefits of Doing a Clinical Attachment

A clinical attachment is not necessary for NHS applications or GMC registration. However, that doesn’t negate its benefits, which include the following:

Familiarity with the NHS

When you do a clinical attachment, you familiarise yourself with the ethical and cultural parts of the NHS, which are handy to know before you work there. In general, you’ll feel far more confident starting an NHS job after completing a clinical attachment.

Gain experience in a speciality

It’s good for any doctor to gain experience in a speciality – even if they don’t pursue that speciality down the line. As an IMG, doing a clinical attachment allows you to observe a speciality with zero commitment. It might tell you that it’s a speciality for you, or, you might learn that you should pursue another route. Either way, it’s a valuable experience.

Improved language skills

Getting used to a new language can be difficult for IMGs. Even if you’re fluent in English, the phrases and accents might take some time to get used to. A clinical attachment will familiarise you with that before you start your NHS role.

As you can see, clinical attachments greatly benefit you as an IMG; it’s all about attaining knowledge and familiarising yourself with how the NHS runs.

You don’t need GMC registration to do a clinical attachment. However, you will need the following:

  • Proof of ID
  • Compete a Criminal Records Check
  • Proof of English Language Skills (IELTS or OET)
  • Occupational Health Clearance
  • Attend the Doctor’s Induction Day

To travel to the UK, you’ll also need to apply for a standard visit visa, which allows you to stay in the UK for up to six months.

How to Apply for a Clinical Attachment

There is no set route for applying for a clinical attachment. That can make finding one tricky, as there is no standard online application. That doesn’t mean you won’t be able to find one, though – you just need to do some digging!

Your first step should be to search for NHS hospitals and then use their search bars to type “clinical attachment”. If they have any clinical attachment vacancies available, they should show up on the website.

Of course, not every NHS trust will have results for clinical attachments. They may still take IMGs on for them, though. Many IMGs find luck by reaching out to consultants or NHS HR departments via email. If you have any friends and family in the UK working for the NHS, you could ask them about any potential openings.

Once you find a clinical attachment, you’ll usually be instructed to fill in a form that will ask for the following:

  • Personal details
  • Medical background
  • Clinical experience
  • Why you want to do the clinical attachment
  • Qualifications
  • An attached CV

What to Expect from a Clinical Attachment

If you get on a clinical attachment, you’ll likely wonder what to expect. Generally, clinical attachments only last between two and six weeks, so you won’t be in the UK that long. On your first day, you’ll be given your aims and objectives, which will likely include:

  • Familiarising yourself with NHS policies
  • Improving communication skills
  • Understanding legal frameworks

You’ll be given a brief induction on your first day, too, which will involve integrating you into the team.

The day-to-day structure of your clinical attachment will depend on your chosen speciality. You might spend a couple of days in one department, and then move on to another. Throughout the experience, you’ll supervise and shadow your assigned consultant, as well as attend regular meetings where you’ll go over your objectives.

To make your clinical attachment as rewarding as possible, it’s important to integrate fully into the team, make the most of meetings, ask questions, and be curious. The experience is for you to learn, after all! Plus, the more effort you put into making a good impression, the better reference you’ll get from your consultant at the end.

When Should You Do the Clinical Attachment?

You might wonder about the best time to do the clinical attachment. We recommend doing the clinical attachment at the same time as your PLAB part 2 test (if applicable). That’s because the PLAB part 2 requires you to travel to the UK. By doing the test at the same time as the clinical attachment, you will only have to use one visa to complete both.

While clinical attachments are not necessary for visas, GMC registration , or NHS applications, they still provide an excellent experience to IMGs. In just a few short weeks of observation, you will learn a lot, and you’ll feel far more confident starting your NHS post in the future.

An introduction to life on the wards

After two or more years of learning medicine by osmosis from the back of the lecture theatre, the transition to clinical years can be startling. For the first time in your life as a student, you are set free—more or less—from the shackles of timetables and will now be attached to a group of doctors working on a specific ward, commonly known as a firm or attachment. During this time you are expected to be a self directed learner on the wards, but this change isn’t easy to make. Let loose in a hospital, brimming with enthusiasm but without direction, it can feel like there is an overwhelming amount—and yet, at the same time, nothing—to do. You may feel under pressure to arrive at crack of dawn each day, rushing from ward to clinic and only stopping when you realise, 12 hours later, that you haven’t eaten all day. On the other hand, with nobody watching, the temptation may be stronger to slip away to the safety of the common room as soon as the ward round ends. No doubt you will also have a long list of clinical competencies to be ticked off, from basic ward work to more specialist tasks in anaesthesia and surgery. And while clinicians and teaching staff will support you as best they can, patients will be their priority before your learning requirements. So, how can you make the most of your time on the wards? We offer some tips on ways to navigate your first clinical attachment with confidence in this self-directed, yet pressured, environment.

Application Process for Clinical Attachment: • Applicants must fill up the form of the clinical attachment. The Requirements are:    o Curriculum Vitae (CV).    o Three (3) Recent Recommendation Letters at least.    o A Copy of MBBS Certificate.    o A Copy of Transcript of Records.    o A Copy of Internship Certificate.    o A Copy of the Board Certificate.    o A Copy of Training Courses.    o A Copy of National ID for Saudis / Iqama for non-Saudis.    o Application Letter.    o Payment of 1000 SR for non-Saudi. • The application will be reviewed by the PGD Specialist before sending it to the Program Directors of each department for approval. • Once approved, we will send an official ACCEPTANCE LETTER which will include list of requirements to the applicant. They should present their National ID/Iqama for non-Saudi with a one Personal (1) photo to get their temporary KFMC ID badge.
